Brian Cloch

Mr. Cloch is a serial entrepreneur and visionary with over 38 years of experience across development, ownership, management, and administration of healthcare organizations. He tailors health services for older adults across clinic, community, home, and post-acute care settings. When Brian sees something that does not work, he finds a solution. Brian began his career as an assistant administrator in a skilled nursing facility in 1983 and has since launched many successful companies that serve older adults. His work requires high-level collaboration with community and government agencies, and he has served many roles throughout his career including founder and current board member of the Affordable Assisted Living Coalition, former chairman of the Illinois Department of Professional Regulation Nursing Home Administrator licensing board, and current member of the Medicaid 42 Advisory Committee. People call Brian a “visionary” because he solves problems before others even know they exist. Brian worked closely with the state of Illinois to create what is today respected nationally as the largest and best affordable assisted living program. Brian has a history for working with governments at local and state levels to create new innovative models of care that increase quality and reduce cost.

Transitional Care Management (TCM) was founded by Brian Cloch in 2009. With nearly 30 years of experience in developing, financing, and managing senior living and rehabilitation facilities, Cloch has developed a winning plan and a proven team for building and managing skilled nursing and transitional care facilities.

Transfers, Cancellations and Refunds Policy

Registrants may transfer their registration to another person at any time by notifying Gabby Palmieri ([email protected]) in writing. Cancellations must be made in writing to Lindsey Beck ([email protected]). Refunds minus a 20% service charge are available for cancellations received in writing 45 days or more prior to the start of the conference. No refunds are available thereafter.
