Ivette Granja, RN, BSN

Ivette S. Granja currently serves as the Vice President of Utilization Management at Integrated Home Care Services (IHCS). IHCS is a leading home care benefits manager and home care provider offering home health, home infusion, and durable medical equipment services to over 2.4 million patients across 12 states and Puerto Rico. She leads a team of clinicians that coordinate the delivery of value-based home care solutions that help patients, providers, and health plans achieve their care goals in the home.

Ms. Granja has over 25 years experience working in health plans in a variety of roles within utilization management, population health management and business development. In her roles she has led teams through successive CMS audits and NCQA accreditations and surveys. Prior to joining IHCS in 2021, Ivette served as Vice President of Care Management at AvMed, Florida’s largest not-for-profit health plan. She has also worked at other health plans, such as, Simply Healthcare (acquired by Elevance), United Healthcare and Florida Blue.

Ms. Granja attended the University of Miami where she obtained her Bachelors in Science of Nursing degree. Her clinical experience was obtained working in orthopedic/neurosurgical and inpatient rehabilitation for 7 years. As a 14 year cancer survivor, Ivette volunteers her time to provide one-on-one emotional support to patients and family members undergoing cancer treatment.
