Sarah Mills is the founding Executive Director of Partnerships for Advancing Community Health (PACH), a statewide nonprofit organization advancing equity-driven community care through Ohio’s network of Certified Pathways Community HUBs. With over 15 years of experience in nonprofit operations and implementation leadership, Sarah supports efforts to strengthen the community health workforce, improve care coordination across sectors, and enhance data and reimbursement infrastructure for CHW-led services.
At PACH, she has launched statewide peer support networks for Community Health Workers (CHWs), secured significant grant funding for workforce and data integration initiatives, and convened more than 170 stakeholders for the organization’s inaugural statewide conference. She also led the successful connection of HUB data systems to Ohio’s Health Information Exchange (HIE), enabling real-time hospital alerts for CHWs. She now leads efforts to standardize and share Community Health Worker documentation through Continuity of Care Documents (C-CDA), helping to ensure that CHW contributions are integrated into the broader care team’s records.
Previously, as CEO of Health Care Access Now, Sarah built on her predecessor’s foundation by continuing and expanding contracting strategies with Ohio’s Medicaid Managed Care Plans. During her tenure, the organization grew its services and increased the number of CHWs, even during the challenges of the COVID-19 pandemic.
Sarah brings deep expertise in nonprofit operations, systems design, and implementation—always grounded in community priorities. She holds a Master’s in Public Administration with a focus on health services from the University of Missouri-Kansas City.
